Explanation Of The Fields On This Form

The following chart lists each of the fields that are on the form, which of these are required and how we use all of this information. Please note that:

When you are adding a person, please enter all of the fields.

When updating information about a person, enter the required fields and then only the others that need to be updated. All of the other fields on the Directory for them will remain as is.

While we don't require their phone, we would use it only if all other attempts to contact them have failed. It is assumed that this person is to be contacted via you, their Cyber-buddy. If you don't want to be their Cyber-Buddy, indicate so on the form and we'll assign someone else to this person, or we may contact them via regular mail.
