The following chart lists each of the fields that are on the form, which of these are required and how we use all of this information. Please note that:

When you are adding yourself, please enter all of the fields.

When updating information about yourself, enter the required fields and then only the fields that need to be updated. All of the other fields on the Directory for you will remain as is.
